End-of-life care allows residents and family members to finish important tasks, saying final good-byes, cherishing family relationships and completing a spiritual journey. Many people use this time to strengthen bonds with people they love and to enhance life's meaning in their final passage.
As part of a team of healthcare professionals including physician, hospice and/or social workers, Discovery Care Centre emphasizes:
- Control of pain and other symptoms through medication and alternative methods
- Medical care focused on patient comfort, organized into a personalized care plan including frequent assessment, equipment, medication and supplies as needed
- Assistance in documenting and communicating the patient's wishes for medical care, including Advance Directives, Living Wills and/or Durable Powers of Attorney
- Coordinating spiritual and bereavement support to help patients and families come to terms with dying.
